Snail Mail: Pay by check for kids, teens and CIT Level 1 with snail mail enrollment. Download and complete Enrollment Form and Fee Worksheet. New CITs (Level 1) use CIT Enrollment Form and general Fee Worksheet. Send all materials by snail mail to: 1645 4th Street, Berkeley, 94710. Confirmation sent by e-mail.

In Person: Drop off completed Enrollment Form and Fee Worksheet to our West Berkeley office at 1645 4th Street, Berkeley, 94710. (New CITs see above.) Confirmation sent by e-mail.
